Service Description
Basic Letter :$25 • Includes simple notes like school excuses or brief confirmations. • Detailed Letter or Form (multi-section or specialty-specific): $50 Includes documents that require time to review, fill out, or tailor based on labs or diagnosis Examples of Supported Requests: • Work or School Excuse Notes • Return-to-Work Clearance • Medical Necessity Letters (for therapy, service animals, gym, etc.) • Travel or Flight Clearance • Camp or Sports Participation Forms • Medication Confirmation Turnaround Time • Standard: 24–48 hours • Rush Option (Same Day): +$15 Disclaimers • Not all requests can be fulfilled if they fall outside the scope of virtual care or lack medical justification. • Forms must be provided in advance. • We reserve the right to decline requests that require in-person evaluation or specialist input.
We understand life happens. If you need to cancel or reschedule please notify us at least 4 hours in advance. Cancellations made less than 4 hours before appointment may result in $15 late cancellation fee. No-shows or missed appointments may be subject to $15 no-show fee.
AffordaCare Virtual does not offer refunds for completed visits. If you are unsatisfied with your care, please reach out—we want to make it right.
