Personalized labs to give us a clear picture of your overall health. Labs included (CBC, CMP, Lipid Panel, TSH, Hemoglobin a1c). Customized Health Plan Get a personalized wellness plan based on your results and visit- whether your focused on prevention or managing chronic conditions. Discounted Follow up visit Stay on track with a follow up appointment at a special new patient rate. Perfect For: Anyone who hasnt seen a provider in a while, busy professionals needing flexible care or those just wanting a fresh start with a provider who listens.
We understand life happens. If you need to cancel or reschedule please notify us at least 4 hours in advance. Cancellations made less than 4 hours before appointment may result in $15 late cancellation fee. No-shows or missed appointments may be subject to $15 no-show fee.
If you are more than 10 minutes late, your visit may be canceled and treated as a no-show. We recommend logging in a few minutes early to ensure everything is working properly.
